Replacement/Revised Paperwork

*Should students lose/misplace their paperwork which are REQUIRED for LVMPD appointments, students will be able to obtain additional copies. Each set of copies will require a $30 reprocessing fee per Student which must be paid in full PRIOR to paperwork being re-processed.
